To enroll in the Yoga Class, you must download and complete the EWP Yoga Class Registration Form.

You must complete the Risk Assessment for Yoga Class Form. Please download and complete the form and mail it to: Employee Wellness Program, KHS-121.

You must also complete the Consent for Yoga Class Form. Please download and complete the form and mail it to: Employee Wellness Program, KHS-121.
