To enroll in the Pilates Mat Class, you must download and complete the EWP Pilates Mat Class Registration Form.

You must complete the Risk Assessment for Pilates Mat Class Form. Please download and complete the form and mail it to: Employee Wellness Program, KHS-121.

You must also complete the Consent for Pilates Mat Class Form. Please download and complete the form and mail it to: Employee Wellness Program, KHS-121.
