Aerobic Fitness Test (Graded Exercise Test)

Each potential participant must complete an Exercise Risk Assessment Form to determine their level of risk. An Informed Consent Form must also be completed. And in some cases, a Medical Clearance Form may also be required.

Please download and print the appropriate forms, complete them, and bring them with you to your appointment. Pay-ment by check (payable to Employee Wellness Program) is required at the time of the test.
