To enroll in the Circuit Training Class, you must download and complete the EWP Circuit Training Class Registration Form.

You must complete the Risk Assessment for Circuit Training Class Form. Please download and complete the form and mail it to: Employee Wellness Program, KHS-121.

You must also complete the Consent for Circuit Training Class Form. Please download and complete the form and mail it to: Employee Wellness Program, KHS-121.
